Supporting People at Home - Intermediate Care and Reablement Strategy

Summary

This report will introduces the joint Intermediate Care and Reablement Strategy between the Council and NHS Medway Care Commissioning Group (Medway CCG) and provide information on future implications.

 

Developed to follow a range of national Health and Social Care policies, it is  aligned with and includes a range of local strategies to support Medway residents to live independently and to stay in their own homes for as long as possible.

 

Stakeholders have been consulted in producing the strategy and will continue to be consulted as aspects of the strategy are implemented.  The Strategy is being considered by the Health and Adult Social Care Overview and Scrutiny Committee on 11 August 2015 and Health and Wellbeing Board on 15 September 2015.
